DVMS is happy to refill your prescriptions as needed for pick up at our practice. We do not offer mail order refills at this time.
If your prescriptions were filled at an outside pharmacy such as a retail or compounding pharmacy, please contact them directly. If they need any clarification they will contact our office. If your prescriptions were filled at DVMS, please fill out the following form. To submit refill requests for more than one pet, please complete this form separately for each animal.
We require 24 hours to fill refill requests submitted Monday through Thursday. Requests submitted on Friday may not be filled until Monday. If you have any questions, please contact us at 480-635-1110EXT. 7 during normal business hours.
